Submit a Report

Please use this form to report any harm (harrassment, abuse) that you have experienced and/or witnessed as a part of Elevation church.  Your concerns are valued by the Elevation leadership team, and we are grieved that you, or someone else you know, has been harmed. It is our desire that everyone participating in the life of our community feels safe and listened to. If you have experienced harmful or abusive behaviour from a leader, or member of our congregation we want to hear your story and take appropriate action. We recognize that it may be difficult to share your experiences, and we commit to standing by you. 

What can you expect from us? 

  1. A response.
    A member of the team you select below will be in touch with you in a timely manner to discuss next steps.
  2. Confidentiality.
    Please be assured that in most cases, any and all information you share is kept strictly confidential, and shared only with the appropriate members of the Elevation board and pastoral team. Situations where we would be obligated to break this confidentiality include:
    • If there is a disclosure or suspicion of abuse involving a minor.
    • If the victim is a vulnerable adult.
    • If it is required by law or a court order (if a legal investigation is required, for example)
    • If there is a threat of harm to self or others

Why can this form not be anonymous?

We are unable to receive anonymous reports of harm and abuse. We recognize that coming forward takes courage, and that you are putting a lot of trust in us to hold your stories carefully. We ask for your name and contact information for the following reasons:

  • So we can follow up with care and support.
  • So we can conduct a fair and thorough review.
  • So we can prevent patterns of harm from continuing.

We have a zero-tolerance policy for retaliation. If you experience any negative consequences for reporting, please notify us immediately.

Where can I bring concerns if I don’t feel ready or safe to use this form? 

If you have concerns, but are not ready to submit a report, we encourage you to speak with a trusted board member and ask them to pass along your concerns and/or experience.

If you do not feel comfortable including your name and contact information in this report for safety reasons, please contact the appropriate legal authorities or services listed below so that they can support you through any next steps of reporting.
